Enrollment for the 2026-2027 school year is now OPEN!
If you are new to our district, we encourage you to call the school at:
620-672-5651 prior to filling out any online forms.
Once you’ve called and gotten the go ahead, please scroll down this page to find everything you need to enroll your student/s!
Returning Student Enrollment Procedure
We are excited for another year! Please complete the following:
- Returning Student Enrollment Form
- View the School Supply Lists
- We encourage all families to view the following letter, and complete the Application for Free and Reduced Price School Meals
- Visit the Nurse's Corner to see if any other forms are applicable to your child
- PreK & Kindergarten parents must complete two Ages & Stages questionnaires with your child: Ages and Stages Questionaire-2 (social/emotional development) and Ages and Stages Questionaire-3 (communication, gross motor, fine motor, problem solving, and personal social skills).
- Optional- Junior & Senior Military Opt-Out Form

- New Student Enrollment Form
- Provide a copy of a legal birth certificate to the school
- Provide a completed medical form to the school
- View the School Supply Lists
- We encourage all families to view the following letter, and complete the Application for Free and Reduced Price School Meals

If your student is NEW to Skyline, arrangements MUST be made with Skyline School Nurse Marie Hayes by emailing mhayes@skylineschools.org or calling the front office at 620-672-5651 to provide the following physical forms PRIOR to their first day of school: -Birth Certificate (legal copy) -Immunization Records (found on link) -Health Assessment/Physical (found on link)
